20yrs female pt wt39kgs.c/o severe white thick discharge since 3yrs , severe back pain with regular m.c and stomach ache during periods.tiredness. becaz of that always sleeping at home.not going to college properly. 3months she took treatment now no white discharge, no tiredness, no back pain. Used dhanwantaram kashayam,gandharwahasta kashayam,lodhrasavam,pushyanuga choorna, kuberaksha vati 1tab per day.Regular to college .to day pt mother saying thanks to me .

Regarding  Pittashmari , guruji Venugopal rao posted long before  one Ekamulika prayoga- Kokilaksha kshara. Good results as it does ksharana but pitta rakta shamaka. Ghrita kalpana of specific dravya which acts on pittashaya .liver also beneficial I think

As Vd Rajyalakshmi, and Vd vinaya has said that should be followed in general. What I do in my practice is ask the parents or elders in their house what they will do as a custom in their family, if there is any viruddahara or virudda kriya then advise not to do that in present situation. I will first advice what not to do i. e., head bath during first three days of Mc, over exhaustion, stale, refrigerated food, untimely food, over eating, traveling, strenuous work etc., advise, fresh food which are mootralas as well as anulomana food what ever available in that season. Advise Suji preparations like ksheera etc., preparations from plantain rinf

Plantain plant rind (baale dindu in kannada) on fourth day and fifth day give haridra kalka in empty stomach so that excess bleeding can be avoided. If low back or abdominal pain is there then jeera and pepper (2gms of jeera and one or two pepper seeds fried, powdered and make kashaya decant it and add ghee to it and drink, from fifth day till next Mc abyanga with til oil or if they are very lean then shatavari Taila. Either ashokarista, lodra kashaya or lodrasava depending on her prakruthi so that vaata pitta anulomana will be proper. If she has any other complaints treat accordingly so that chances of cure are more during that time.

We have direct references on pittashmari in texts so theoretical background is not a problem.

What is important is , are we able to apply the same and get successful results in practice?
If so up to what size calculus can we take up?

The other day, I had a patient aged 70 years enquiring about Ayurvedic treatment for her 29mm size single gall stone.
She had two episodes of severe pain in the abdomen but wanted to avoid surgery that was advised.
Her general parameters were OK.
I had absolutely no second thoughts over asking her to go for surgery soon.

Susruta suggests paneeya ksharam in pittashmari. I gave yavakshara with arogyavardhani to a patient once but  stone size did not decrease, although, digestion and general condition improved. pain also decresed. Should I have added Hazrul yahood bhasma also?

Just like mutrashmari chikitsa.. except arogyavardhini

For prevention of mutrashmari I generally suggest regular intake of lemon.
Is it helpful in pittashmari also?
Lemon is amla padartha.
Which will be better ?
Kshara or amla.
Also ksharas cannot be used for long?
How long to use it?

As gall stones are formed due to concentration and stagnation of bile juice which is cholesterol based, can we achieve better results by using medopachan dravyas

Ksharas, Arogyavardhani, varunadi kashayam etc. are helpful for ksharana, lekhana and sarana of bile , no doubt

Will adding guggulu help?

I request subject experts and MD scholars to give their inputs. Many young Vaidyas eager to practice authentic Ayurveda are waiting for a little guidance.

vacha rubbed over stone 4-5 times with water yields a very small quantity of paste . It is mixed with honey and applied over tongue in clot infarctions repeatedly but dosage should be taken care of as excess vacha can lead to vomiting.
Its our late Guruji Sripathi Rao's prescription in emergencies.

I too suggest it to patients who have developed clots from haemorrhages

Not only stress, I feel thyroid problems which are in epidemic proportions are the result of dushi  visha. Chlorinated water, fluorine in water, pesticides, unnecessary iodine in salt, pollution. That is why we find vyadhi Sankara.

Excess iron supplementation (in pregnancy) is a cause of hypothyroidism too

Dear Vaidyas, I would like you all to help me with my mother's back problems.
Age 74yrs. Known diabetic since 15years. Known hypertensive since 10 years. Doesn't take any medicine for Diabetes. On strict diet control and takes karavellaka kashaya for Diabetes. But on allopathic medication for Hypertension. Sorry can't remember the medicine name for hypertension.
She has been having back ache since 2 years. Aggravates on sleeping. She cannot sleep at night due to pain. So starts walking. Feels better.  I had got her matra basti 2yrs back with Mahanarayana taila from my friends at Mangaluru. She got 95% relief. Stiffness of back reduced. She could easily get up from squatting position. So she went back to Hyderabad. Recently she started getting same complaints. I got her treated at my hospital at Moodbidri. This time she was treated with
1. Sthanika parisheka right knee and below with Pinda taila
2. Upnaha with
       Jadamansyadi choorna.          +     Godhuma+Pinda taila +
        Saindhava +
        Dashamoola quatha
3. Matra basti with Guggulu Tiktaka Kashaya
4. Trayodashanga Guggulu.

She completed 7 days treatment.
But night sleep still disturbed due to pain in lumbosacral region and pain from knee downward in right leg.
Now Trayodashanga Guggulu stopped and Gandha taila started since one week. She has improved a lot after taking Gandha taila. But still complains of stiffness of lumbosacral region.
I would be grateful if you could suggest something which will release stiffness of back. She is returning to Hyderabad on 13th of this month.

Karavellaka may be causing severe vata prakopa in her. Though blood sugar levels are under control other Vataja problems may aggravate by intake of Karavellaka regularly. First stop that and give any brumhana mehahara compositions like vasantha kusumakara etc so that vata shoola shamana happens simultaneously.

For hypothyroidism
Kanchanar guggulu and Varadi , Varunadi kashayas  are helpful if condition is detected early. Along with yoga exercises can be reversed in some cases.

For hyperthyroidism, Narasimha rasayan, ashwagangha lehyam, Brahma rasayan am etc. work

A proper discussion will be very useful to all.

We can consider listing common symptoms of hypothyroidism, like weight gain, fluid retention, cold intolerance, hairfall, shukra and artava vikrutis and maybe analyse the doshas involved, dushyas affected, nidana, samprapti in Ayurvedic lines and analyse how Ayurvedic treatment can be planned.

In hypo thyroidism we can also give Hamsa pada kashayam (kotakkal medicine) and godanti bhasma.

But in thyroid problem we know..root cause is hormonal imbalance...

Most of the hypothyroid cases which I have come across is in females with mc imbalances, diagnosed during pregnancy first trimester blood test and on allopathic treatment. That time they are worried about pregnancy complications and do not want to take any risk and are on allopathic treatment and later cannot withdraw allopathic medicines but still suffer from hypothyroid symptoms esp related to mc and anxiety issues. Please discuss on this topic

I mean in most hypothyroid cases the causitive factor is stress ,. Stress harmones constantly hit the thyroid gland and its functioning is slowed down so the kashayams like varadi(trip hala) varanadhi and hamsapadhadhi will remove these toxins or stress harmones from blood so automatically it's functioning will improve.

And also iam using godanti bhasma
Gorochanadhi tablets
Dhan want ram (101) drops( taken in nostrils ,i mean nasya karma ) to treat hypothyroidism. Most of the cases are responding successfully.

Bramari pranayama  is most imp exercise to correct thyroid functioning.

A 44 yr old male, with complaint of vertigo, got an MRi of brain done, report suggests heterogeneously enhancing soft tissue lesion arising in relation to anterior pituitary- likely to be pituitary adenoma. In appearance the hands and face of the patient seem to have broadened. Serum growth hormone level has risen  13.071 H Ng/ml (normal- 0.003-0.971), in thyroid profile FT4 24.53 H pmol/l 12-22 rest is within normal limits. Sleep disturbed. Bowel bladder appetite- normal. Doctors at DMC, Ludhiana have refused to operate. What can be given?

Shloka 4 on prakrita kapha

श्लेष्मा स्धिरत्वं
Shleshma is for stability

Applied aspects - Stability is possible due to healthy growth and regeneration of tissues.
When kapha is vikruta this aspect is affected.
A child is in kapha kala of life.
A seven or eight year old kid who chronically presents with running nose or tonsillitis or similar kapha vikruti also frequently is seen to have a slow or stunted growth.
Unless kapha vikruti  is treated, growth and stability cannot be restored.

Theory - स्निग्धत्वं - lubrication

Applied aspects - The lubrication of the entire body is dependant on kapha.
Look for complaints of rukshatva described by patient especially of the five kapha subtypes. Supporting kapha must always be accompanied with protection of agni.
If we take into consideration , the snehas, ghritham and taila
tila taila  is vatashamak, kapha shamak, and pitta vardhak.
So it can be used by obese people.
Ghee is pitta shamak, vata shamak, and kaphakara. So it has more properties of snigdhatvam.

Theory - संधिबंध - compactness of joints

Applied aspects - Sandhibandha  is the reason for posture, capacity to work.
A good example to understand this aspect of kapha is from the relationship between vertebrae and inter vertebral discs.
Disc degeneration is kapha kshaya and that leads to vata vriddhi.
So while we treat postural and degenerative diseases as vata vikrutis due to the complaints of pain, stiffness etc. we must remember that the nidana is actually kapha kshaya which if treated gives lasting relief.

Theory -
श्लेष्मा - क्षमा
Kshama literally is forgiveness

Applied aspects - In treatment it means capacity to withstand both physically and mentally.
Physical kshama is therefore , resilience, tolerance, resistance . When does a person require these qualities?
Ans. In adversities.
Whatever causes hardship to the body, change of seasons , pollution, germs, stress and strain etc. can make a person vulnerable to diseases. It is prakrita kapha that gives resistance power.
When this power becomes low it is kapha correction that needs to be attempted of, course bearing in mind the agni.

Psychologically also, kshama or forgiveness or mental resilience or letting go of bad experiences can be affected if kapha is under vikruta condition. So in such cases kapha correction and pacification of vata and pitta is helpful.

The summary is Ayurveda is uncomplicated. We only need to have a proper understanding of the prakrita conditions of the doshas to understand their manifestation in pathology or vikruti.

Patients that come to us, rarely come with clear cut symptoms.
Many times we find juxtaposition of two or more diseases in a person. Let us not worry about knowing the name of the disease. Understanding dosha  manifestation, dushyas involved, prakriti, kala desha  etc. we can successfully treat all conditions.
Charaka also said, a physician need not be ashamed about not knowing the name of every disease. It is just not possible. But when doshas are brought into equilibrium the disease is taken care of.

Question time friends.

Why is pippali which is ushna and teekshna useful in amlapitta?

Geetha Rani:
Because pippali is madhura vipaka. In alma pitta drava roopi pitta vruddhi is there. Ushna guna of pippali helps in decreasing dravata. Teekshna???

Rishi Vashishth:
First we need to understand Amla pitta and pippli individually, site of the disease is grahani or adha amashaya, in which the amlatva of pitta is deranged. Pitta has both Amla and katu guna, but in Amla pitta amlatva has increased ,Amla pitta might be urdhvaga or adhoga, or it has been said it may be vatanubandh or kapha anubandha.... When kapha anubandha is there...the amlatva is effecting above the grahani i.e urdhva amashaya (site of kapha) also, and when vata anubandha is there it is effecting below the grahani ie pakvashaya also.... Which is y in urdhva Amla pitta virechana is indicated and in adhoga Amla pitta vamana is indicated..... Pippli is  madhur vipaka( no presence of amlatva) and at karma level pippli is vamanopaga( not vamaka) and mild laxative in nature....(laxatives are indicated in urdhva Amla  pitta as i mentioned before)so it seems it is at the karma level, and not due to its doshic action

So it supports sasneha agni. As Sharmaji pointed out it is anushna.

Shloka no.3 on prakrita Pitta

Theory  - पित्तं पक्ति , ऊष्मा, दर्शनै:

Applied aspects - Pitta is responsible for pachan ie  any transformation or conversion is the function of pitta.
When agni is sasneha , it is pitta. So cannot equate agni with pitta entirely.
If we keep in mind the Alpa sneha guna  of pitta, it opens up many possibilities in practice.
A little amount of goghritha is deepan. Hingwastaka churnam works best with goghritha for deepan and pachan as it supports prakrita pitta.

Pippali churna  with goghritha is effective in amlapitta as this combination supports prakrita jatharagni . It does not suppress healthy appetite like the antacids.

In agni alpata kapha is increased. So in agnimandya, kaphahara and pitta balakara aushadhis have to be given.
Slightly ushna virya aushadhis will work here.

If we take into account each prakrita property of dosha we shall be able to identify the vikruta behaviour. For instance, in Parkinson's chala guna is aggravated and sthira guna is reduced. That is why atmagupta which has vajikara and sthira guna is effective in such cases.

पक्ति or conversion is linked to enzymes and hormones. Hence even if secretions are the functions of kapha, in hormonal imbalance prakrit pitta functions will have to be evaluated and rectified

Theory - ऊष्मा
Maintenance of body temperature.

Applied aspect - Ashraya of pitta is sveda. Sveda nissarana brings down ushma. So giving drugs with vatsanabha , relieves fever by inducing sweat

Theory - दर्शन - vision

Applied - vision problems are mostly pitta vikrutis. Pitta is best pacified with ghritha  so use ghrita as anupana  for vision related illnesses due to pitta.

Theory - क्षुधा -  appetite

not feeling hungry is a pitta vikruti. It has to be treated with ushna virya deepana drugs but ensure that some snigdhata is maintained to support prakrita kapha .

Theory - तृट् - thirst

Applied - In case of both lack of thirst ( sasneha and slightly ushna drugs should be given)and increased thirst( cooling drugs should be given) it is pitta that has to be corrected.

Theory - रुचि  - taste and healthy appetite.
In arochaka sasneha pitta vardhaka will help.

प्रभा - prakrit pitta is responsible for glowing complexion.
In discolouration pitta shamaka and rakta prasadana are useful.
As doshas are in anubandha, identify main dosha, anubandha dosha and then treat.
Neelika is a prabha vikruti but its a vata vyadhi with pitta anubandha. So treatment will have to planned for vata, rakta and pitta.
If we observe the complexion promoters , manjishtha, yashthimadhu or sariva  all of these correct pitta.

Theory - शौर्य - courage and valour are prakrita pitta gunas

In vikruti if excess of aggression is seen pittashamakas have to be given.
Negative emotions also lead to prakopa of doshas such as
क्रोधात् पित्तं ,भयात् वायु:
So these nidanas also give a clue about affected dosha

Again sasneha aspect is seen.
In roughness or kathinya always look for pitta shamana  also.

Ayurveda Prakriti - The Three Doshas, have been Correlated with Genomic Structures in a Research Study 

A genome-wide study indicated certain correlation between Ayurvedic prakriti classifications with genomic diversity.  A study on the subject has been done by the Centre for Cellular & Molecular Biology (CCMB), Hyderabad a premier research organization of Council for Scientific and Industrial Research with the collaboration of other national research institutes.


A research team at the CCMB under the leadership of Dr. Thangaraj, has taken up the mega program. Well-trained Ayurvedic physicians screened about 3400 people and the same sets of people were also screened by software called AyuSoft developed by C-DAC, Bangalore. 


Ayurvedic physicians believe that there are three doshas or biological energies / humors found in the human body.  Ancient Indians believed that everything that we see is made up of five elements – Space, Air, Fire, Water and Earth.  Vata is related to elements of space and air; pittha is related to elements of fire and water and kapha is related to elements of water and earth.  Each individual would have different levels of these three doshas, hence the diversity.  However, each person can be classified, belonging to one or the other type, if one of the doshas predominates.


People whose Prakriti was in concurrence between the assessment by the Ayurvedic physician and by AyuSoft were recruited for the study.  Their blood samples were collected by respective participating labs. Isolation of DNA and genomic studies were carried out at CCMB using Affymetrix 6.0 SNP chip. This chip brings out single nucleotide difference in the genome among the tested samples.  When the data is plotted, interestingly they fell into three groups, establishing the molecular basis for ancient classification. 


Whether such phenotypic classification has any molecular basis has been a matter of debate for some time.  A few groups attempted to answer this question and found some correlation when they looked at one or two specific genes.  However, the association of genomic variations with prakriti classification was lacking.


This is the first genome-wide study to establish such correlation between Ayurvedic prakriti classifications with genomic diversity.  Analysis of these single nucleotide polymorphisms (SNPs) revealed that about 52 genes might be responsible for specifying the individual's doshas or prakritis.  Dr. Thangaraj and his group has taken this study further and attempted to see if the samples collected randomly, without any information on their prakriti, would also fall into three groups after the analysis based on the 52 SNPs, which appear to be important.


Director-CCMB Dr. Ch. Mohan Rao said that this study would help to identify the prakriti of a person based on her/his genome. “This is a major breakthrough linking our ancient wisdom with modern science”. He also said that this work will inspire many more such studies. These studies should eventually lead to establishing Ayurveda on sound footing along with modern medicine.

Question on GST : I am a practicing Clinical Doctor. I purchase medicines from suppliers and in the process pay them GST (previously called as VAT) on the bills received from them. I then dispense medicines to my patients and issue them receipts including consultation+dispensing fees. It goes without saying that the cost of medicines so dispensed to medicines includes the GST component too. Am I covered under the GST Act ? To what extent are healthcare services covered under the GST Act ? Are healthcare products covered under the GST Act ? How are products different from services as per the GST Act ? Do I have to register for a GST number under the GST Act, 2016 ? Can I recover 'input tax credit' (as paid to suppliers vis-a-vis when collected from patients while dispensing medicines to them) under the GST Act ?

Answer: 1. Healthcare services are exempt from GST under the GST Act, 2016. Healthcare products are not exempted, however. Healthcare products are chargeable. 2. Healthcare services include services rendered for treatment at a healthcare facility. Healthcare prodcts include medicines, instruments, machines, equipments, infrastructure, etc. 3. When a Doctor dispenses medicines, they form part of healthcare services (as per the GST Act) and for a reasonable period of time (as per IMC Rules, 2002), they are nevertheless 'healthcare services' as a whole and are therefore not chargeable to GST. Please note that 'services' are defined under the GST Act, but 'reasonable period' is not defined by either the GST Act, 2016 or the Indian Medical Council Rules, 2002. 4. Dispensing is not same as selling. Selling is not same as dispensing. When a Doctor charges a fees for services rendered that includes fees+medicines as a whole, it is a service (as per GST Act) and called dispensing (as per IMC Rules). It is not called as selling medicines. This is exempted under the GST Act. 5.. However, if a Doctor charges for medicines/other healthcare products solely, these are chargeable for GST. 5. There is no need for Doctors to register under the GST if they are providing only healthcare services. 6. If they are selling, then only one needs to register. Anyways, a Doctor cannot sell medicines unless he is involved in a trade of medicines/healthcare products. 7. In short, GST does not change anything for practicing Doctors. GST is Goods & Services Tax, and replaces many taxes like VAT, etc as they were in use previously. 8. Input tax credit can be availed by Doctors if they are involved in a trade of medicines/healthcare products, not while dispensing medicines to their patients. -

Dosas , dhatus and malas are the basic components of the body. The functions of doshas differ sharply in states of equilibrium and in equilibrium.

Prakrita vata

Theory --- तम् चल:----  responsible for movements
Applied aspect --- The चल गुण is affected in vata disorders.
In paralysis this chala guna  is compromised due to some obstruction ( avarana). So that also must be treated.
In Parkinson's there is excess of chala guna due to increase of vata.
Increase of vata is related to dhatukshaya and decrease of vata is related to avarana. So accordingly treatment must be planned.

Applied aspect -- patient complains of lack of motivation but has bala, energy, treat for vata,

Similarly prakrit vata is responsible for uchvasa nishvasa, cheshta, vegapravrithi

samyagatya cha dhatunam( maintenance of dhatus) and akshanam patavena( maintenance of sense organs .

In respiratory disorders, locomotory disorders treat vata but take into account the other doshas also that are affected.
Maintenance of dhatus requires proper cell division. Unregulated cell division as in cancers indicates vata vitiation. Vata can be equated with the controlling mechanism of the body. Any sense organ disorder needs vata correction

Eg. Why is oil applied to skin?
Sparshanendriya, the skin is the biggest vata organ in the body. Hence thaila abhyanga is advised as a dinacharya to pacify vata

Coming to formation of dhatus, the first dhatu rasa is nothing but kapha swarupa. When pitta joins it , it results  into rakta.
Kshaya of dhatus is vikruta vata manifestation where as proper formation of dhatus is a prakruta function. Eg.Mamsa kshaya or emaciation is a pure vata disorder.

In sandhivata, there is rukshatvam due to kshaya ( vata vriddhi) so oil appln. helps. In Amavata the situation is different. There is obstruction. So most oils cannot be applied because the obstruction can increase.
( Chickungunya can be treated well by applying amavata and vatarakta treatment aspects)

For instance, I just realised I missed  Vegapravrithi as a prakrit function of Vata. What could be the state of Vegas in vikruta vata and how to deal with it?

Want to post a feedback received now. 30 year old patient underwent severe stress and amenorrhea ( complete absence of periods for 8 months) ,facial hair due to it .Periods are regular(28 day cycle) after use of kuberaksha vati from Rasa shala for three months. Some relief also obtained in unwanted hair.
Thanks to team Rasashala and Sharmaji for preparing excellent medicines.

Along with kuberaksha vati also used Sukumara kashayam and kullathadi kashayam.

Dosage of the vati was just one pill a day and the kashayas  10 ml each twice daily. Stress levels due to grief ( death of husband) was handled by the patient with regular walks, meditation and prayers.

We know that we are surrounded by different kinds of microorganisms and also within our body. In Kerala in monsoon season, no matter how much hygiene measures are adopted in monsoon season, there are so many mosquitoes and flies , rats and other rodents who prefer to stay close to us .But all of us do not stay sick all the time by infections. Also when some people are in a home by infection, in the same home, there can be people who are devoid of infections. Some recover very fast while some others get worse and some even get shifted to ICU . Why it happens ??

There are two factors responsible for onset of infections :
1. Microbial factor - Every one is exposed to microbes every time.
2. Host factors- Host factors include general and specific components to fight against microbes. The main general mechanisms are ;
a. The strength of immune system - two types of immune functions happen,a: Vyadhibala virodhitwa: the immunity which fights after the disease manifests,b:Vyadhyutpadaka prathibandhakatwa: the immunity which prevent the infections .The former kind gets activated as soon a microbe invades and starts defence mechanisms then and there itself till the pathogen is completely destroyed/ inactivated . The second immune function helps not only to prevent an infection , but also to prevent attaining the strength of the infection

b. The digestion and metabolism of host : when a microbe attacks the body, the microbial toxins cause impaired assimilation of nutrients in the body making the patient weak, feeling heavy, impaired hunger, constipated( impaired peristalsis), lethargic, inflammatory reactions all over causing body pain etc. So it denotes that both digestion as wella s cellular metabolism in the host is seriously impaired . the rectification of these mechanisms by improving metabolism and neutralisation of toxins immediately rectify the deranged metabolism and digestion and thereby all the inflammatory mechanisms resulted by infection .

c. Drainage mechanisms - The internal drainage ( from within cells to outside) of metabolic waste and microbial toxins is impaired and so accumulation of this in cells happen .When internal drainage is impaired, taking laxatives will never help . Because improperly metabolised waste and toxins is within the cells and not in the colon. During that time, when laxatives are taken , it will worsen the metabolism and internal drainage mechanisms .
d. The mucous barrier - healthy and intact skin and mucous membrane prevents the entry of external microbes or inactivates them as soon as they enter .

(In Dengue fever, some cases lead to DHF because of sustained inflammation by above mechanisms resulting in increased capillary permeability . Ayurvedic treatment guidelines specific to inflammation will prevent as well as check bleeding as the root cause is attended. There are specific guidelines focussed on host factors to be judiciously adopted in all infective fevers which explained very systematically in accordance to nature of intrinsic cause , state and stage in Ayurvedic fever diagnosis and management .)

Ayurveda treats infection by specialised and specific focus guidelines with respect to the above four hostfactors and not by the very reductionist killing vector /microbe policy. Ayurveda is a shastra which has acquired tremendous development in its inbuil;t scientific infrastructure( by intense basic and applied research explained in shastra) from the pre-vedic belief that krimies , bhootas etc cause infections .

So Ayurveda can prevent, detect and cure any sort of infections rampant today in an unimaginably healthier and faster manner. Soon after the patient recover, the patient will forget that he was sick in the past two days owing to attainment of energy, enthusiasm and improved metabolism.

Bhasma as a medicine and the toxicity aspect

 Minerals are essential for the proper function and structure of a cell. However, the human body cannot absorb most of the minerals in their natural (inorganic) form. The body has to recognize them as food. This food is provided by plants that process inorganic minerals from the soil into organic minerals that can be well absorbed by the human body. Concern has been expressed about the poisonous nature of the deified red sulfide of mercury (cinnabar/makardhwaja) and the red oxide of lead. There is a detailed description of their medicinal virtues in the texts22. Lead and mercury exposures of the human body and their harmful effects have been studied26. The absorption of inorganic compounds of mercury in the human body is poor to moderate when administered orally. Its half-life of stay in the blood is 40-60 days. The absorption of organic compounds of mercury is complete and the half-life in this case is about 70 days. Once absorbed, it is mainly distributed into the central nervous system and the kidneys. Its elimination is through excreta. Lead in both its inorganic and organic form is absorbed through the gastrointestinal tract. Once absorbed, it lands up in soft tissues (brain, liver, kidneys). Its half-life of stay in the blood is about 30 days. It is excreted through the kidneys. Chelation therapies have been used to bring down the levels of the toxic metals in patients. However, these require a careful monitoring. It has been expressed that probably the metals while being made into a bhasma get chelated with organic molecules (ligands) obtained from the herbs for these to be better assimilated27. To improve absorptivity and be less reactive in the digestive tract, a metal ion is bonded or chelated to an organic molecule, an amino acid or a hydrolyzed protein (a kind of a Trojan Horse). However, a bhasma when prepared is not a chelate; it tests to be an inorganic compound only. The bhasmas do show up with traces of certain other elements present in them. These come from the impurities, specific treatments and the crucibles and the vessels used15,18. Any organometallic complex compound has to survive the several incinerations at very high temperatures and if at all can be present in extremely small proportions only. Incidentally, minerals taken with a meal get usually automatically chelated in the stomach during digestion28.


 Thus, one has to contend with the simple inorganic compound form of the metal only that’s medicinal value and the attributed pharmacological action is to be scientifically ascertained and its absorption and stay in the body is to be quantitatively determined till it is drained out. Many metal oxides are toxic; in comparison, a sulfide of a metal is generally less toxic compared to its oxide or chloride, being relatively less soluble in the body fluids. As bhasmas are mostly oxides only, the toxicities need to be evaluated. Probably, to check the toxic accumulations, a bhasma is traditionally administered with an anupaana (vehicle) such as honey, milk or butter, etc. This has been empirically known but the knowledge is mostly qualitative. Some of the observations as follows provide more definite indications to the safety factor behind the use of bhasmas compared to the plain salts of the same metals that are toxic in high doses. Recently, some detailed studies have been made of the physicochemical changes in the various stages while preparing tamra (Cu) bhasma by light microscopic, electron scan microscopic, X-ray diffraction and thermal gravimetric techniques using standard protocols29. Initially, copper sulfides and some new metallic complexes are synthesized but eventually it is free of copper. The particle size in the end product reaches colloidal levels. The study also found that the bhasma when given in higher doses to rats does not get absorbed in the gastrointestinal tract but gets excreted. A study on the neurotoxic effect and accumulation of methyl mercury and mercuric sulfide (HgS) in rats indicates that insoluble fo

rm of inorganic HgS

can be absorbed from the G-I tract and that its neurotoxic potency is about one thousand times less than that of soluble MeHg30. In another study, swarna (Au) bhasma was prepared as per the classical texts and was analyzed for its metal contents using atomic absorption spectrometry31. An infrared spectroscopy was performed that revealed it to be free of any organic compounds. Its acute oral administration showed no mortality nor did a chronic administration show any toxicity in experimental animals. Also, the bhasma was found to have antioxidant property.

 Nanosubstances (0.1-100 nm) can have special physical and chemical properties. These are comparable to the size range of biological structures and so are potential substance for manipulation, sensing and detection of biological systems. A single atom is 0.1 to 0.2 nm in size, a strand of DNA is 2 nm wide while a red blood cell is about 7,000 nm. Substances such as metal oxide nanoparticles, nanogold particles and carbon based nanomaterials (fullerenes, nanotubes, nanodiamonds), etc. are being viewed as nanomedicine, with a variety of biological applications, in the diagnosis and treatment of 
a wide variety of diseases. At these scales, many substances can develop toxicity too. Experiments with zinc oxide nanoparticles (nZnO) have shown selective toxicity to different bacterial systems and human 
T lymphocytes32. Gold and its compounds have been known for their therapeutic utility. A standardized colloidal gold preparation (Tyndall's purple, Au°, 
27 nm) has been found to be a far more effective anti-arthritic agent in rats than the sodium aurothiomalate used in the treatment of rheumatoid arthritis33. The administration route was subcutaneous; attempt was to examine if the classic swarna bhasma can also be similarly suitable. Swarna bhasma (42 iterations, 
900 C) was prepared brown red in colour following classic texts (Rasaratnasamuchchya and Sarangdhara-Samhita). An infrared spectrum showed no presence of organic compounds and transmission electron microscope (TEM) measurements revealed average particle size to be 57 nm with globular morphology. An atomic absorption spectroscopy indicated the bhasma to be free of any mercury but having 92% zerovalent gold (Au°) in the bhasma. These nanogold particles should be able to reach the affected site even if administered orally and provide a slow and sustained release of Au (I) ions33. By itself, this is a highly promising area for further studies on treatment of diseases.

 However, an altogether different form of a bhasma, obtained following a classical bhasma process has been used. The swarna bhasmaprocess is a temperature sensitive one  and one must settle it if the swarna bhasma so obtained - predominantly a colloidal metallic gold - is a drug intermediate only since the bhasma process in general is so designed as to produce an oxide, free of organic and metallic particles. The classic texts describe a method involving aqua regia etc. to produce pure and fine yellow coloured gold powder which is used as medicine, in place of gold foils but it does not involve incineration4,24. As an aside, note that colour of a colloid can differ depending on particle size, shape and mutual separation, and refractive index of the medium. Nanogold particles 2-30 nm in size in a colloidal suspension are bright red in colour and their colour changes to purple and blue as the size gets larger as a result of a phenomenon called surface plasmon resonance34. Nanodiamonds are being considered as a potential drug delivery agent that can carry drugs to certain parts of a damaged cell where larger carbon based nanoparticles cannot reach and interestingly the left-over nanodiamonds do not induce inflammation in cells. Their impact on humans and other biological systems is being studied. It has been demonstrated that 2-10 nm nanodiamonds are biocompatible with a variety of cells of different origin and are nontoxic35. Nanodiamonds can for example be produced in a detonation of TNT/RDX 
at 5 Gigapascals and 2,000°C. The nanodia

ltogether from that of a vajra bhasma and so a correspondence can not be made. The bhasma process does not create nanodiamonds as an end product since powdered diamond will be only oxidized away in the numerous incinerations. Nanomedicine is regenerative medicine, for the treatment or repair of tissues and organs, within the targeted cells. The classic bhasma could qualify to be in the new age nanomedicines realm but the correspondence should be done within propriety. Cheap surely but whether therapeutically suitable is another matter that requires detailed studies, particularly of the toxicities.


 The Indian medicines were subjected to critical examination using the standards applied to western medicine where permissible limits to the presence of a toxic heavy metal in a formulation are established1. The work is about metal-contents and not a study of metal toxicities among populations resulting from use of such medicines. Also, the metals chemical forms, which can impact bioavailability and toxicity could not be ascertained. This important aspect has not received the attention it deserves. As inorganic compounds and in prescribed dosages, they are less likely to accumulate in the body. With particle sizes in the micron to near nanoscale range, the absorptivity of the metal bhasmas is presumably better than an ordinary mineral taken in powdered form. Between them, these factors are contrapositive but can also strike a kind of a balance. If the preparations have been done as expounded in the texts, the toxicity aspect is constrained because the compounds have been traditionally in use. All of the bhasma metals are not toxic. The Ayurvedic Formulary of India as also the National Formulary of Unani Medicine list several formulations that contain malla (arsenic) as an active pharmaceutical ingredient. The upper limit to the per diem dosage is 10 mg, for an adult. The formularies do permit usage of As. In very small doses, it is stomachic, general and nervine tonic, alterative, respiratory and sexual stimulant as per the texts22. There are several metal compounds considered poisonous but are permitted to be used in an ISM formulation. There are numerous vaidyas and ISM drug manufacturers who have developed formulations from less controversial drug material, permitted to be used in ISM, and claim positive results. These need a multifaceted study, like any modern medicine today.


 The concerns in the study notwithstanding, use of inorganic chemical substances in medicine has been widespread in the West, well into the 20th century and these have been part of pharmacopoeias1. One can cite here substances such as compounds of mercury, copper sulfate, alum, magnesia, borax and green vitriol, etc. The West has used arsenic trioxide over the past 200 yrs as a part of medicine, including the treatment of cancer. The Food and Drug Administration in 2000 approved arsenic trioxide for the treatment of patients with acute promyelocytic leukemia that is resistant to ATRA. One can see a number of tonics in US, UK, etc. which contain a number of minerals including vanadium, manganese, chromium, selenium, etc.
